Visa forms / Photo requirements

Medical treatment (Schengen Visa, type C)

Please note: for each applicant a complete file is required (including children).

For people undergoing medical treatment or getting medical advice.

Requirements for the patient:

  1. One application form "Antrag auf Erteilung eines Schengen Visums" (Appliation for a Schengen-Visa) duly completed (in German, French, Italian or English) and signed by the applicant in person (consult "Visa forms / Photo requirements" on this website)
  2. Passport which must be valid for at least three months after the date when the holder leaves the Schengen zone and containing at least two empty pages
  3. Copy of the applicant's passport (pages with photo, personal data and signature)
  4. Copies of the last two Schengen visas (if applicable)
  5. 2 identical, recent passport photographs, meeting Schengen requirements (one glued on the application form, the other attached ) (consult "Visa forms / Photo requirements" on this website)
  6. Accident/health insurance valid for the entire stay and accepted in the whole Schengen space (only copy). The insurance must cover the minimum amount of EUR 30’000.00.
  7. Evidence of employment (letter from the employer) and evidence of income (earnings statement or salary slip); for students: student card (original and copy)
  8. A copy (clearly legible) of the invitation letter from the clinic or hospital signed by the doctor confirming the down payment or deposit for the medical services to be rendered
  9. A bank statement showing the account's movements for the last three months, or a bank letter confirming the balance of the account
  10. Proof of transport including return trip (e.g. ticket reservation, only copy)

    For children under 18 years of age: 

    In addition:
  11. Birth certificate (original and copy) and legalized translation (into German, French, Italian or English)
  12. If a child travels alone or is not accompanied by at least one of his/her parents, an authorisation translated (into German, French, Italian or English) and signed by the parents in front of a notary public is required (original and copy). 

    For children registered in parents' passport:
  • One application form "Antrag auf Ausstellung eines Schengen-Visums" (Application for a Schengen-Visa) duly completed and signed by the parent in whose passport the child is registered
  • 2 identical, recent passport photographs, meeting Schengen requirements (one glued on the application form, the other attached) (consult "Visa forms / Photo requirements" on this website)
  • Copy of the passport of the parent with whom the child will travel and in whose passport the child is registered
  • Copy of the page of the passport with the registration of the child
  • Copy and legalized translation (into German, French, Italian or English) of the birth certificate (bring the original and the copy)
  • Accident/health insurance valid for the entire stay and accepted in the whole Schengen space (only copy). The insurance must cover the minimum amount of EUR 30’000.00.

A minimum time of three working days after submission of the visa application (not counting day of deposit) and payment of visa fees is needed for consideration of the visa application.


Requirements for persons accompanying the patient:

  1. One application form "Antrag auf Erteilung eines Schengen Visums" (Application for a Schengen-Visa) duly completed (in German, French, Italian or English) and signed by the applicant in person (consult "Visa forms / Photo requirements" on this website)
  2. Passport which must be valid for at least three months after the date when the holder leaves the Schengen area and containing at least two empty pages
  3. Copy of the applicant's passport (pages with photo, personal data and signature)
  4. Copies of the last two Schengen visas (if applicable)
  5. 2 identical, recent passport photographs, meeting Schengen requirements (one glued on the application form, the other attached) (consult "Visa forms / Photo requirements" on this website)
  6. Accident/health insurance valid for the entire stay and accepted in the whole Schengen space (only copy). The insurance must cover the minimum amount of EUR 30’000.00.

  • Copy of the passport of the parent with whom the child will travel and in whose passport the child is registered (pages with photo, personal data and signature)
  • Copy of the page of passport with the registration of the child
  • Copy and legalized translation (into German, French, Italian or English) of the birth certificate (bring the original and the copy)
  • Accident/health insurance valid for the entire stay and accepted in the whole Schengen space (only copy). The insurance must cover the minimum amount of EUR 30’000.00.

Travellers who intend to apply for a visa for medical reasons or accompanying such people must submit their application in person. They can be represented by a travel agency accredited to the Swiss Embassy/the Swiss Consulate General or by close relatives (spouse, mother, father, daughter, son).  If the documents are submitted by close relatives, the "messenger" has to provide proof of the family ties (copy of birth certificate or marriage certificate: does not need to be legalized or translated) and to show proof of identity.

The Embassy of Switzerland/the Consulate General of Switzerland reserves the right to:

  • request additional documents
  • transmit the application to authorities in Switzerland for decision/approval
  • summon the applicant for an interview
